Amber Sweet is a person who has suppressed everything to the point where she literally feels nothing at all. Nothing bothers her - not the disapproval/neglect of her father, the violence of her brothers, the absence of her mother and the loss of the potential replacement in Marni. It's not for nothing that she's hooked on painkillers - it's blocking that emotional pain she should be feeling. Surgery is, essentially, cutting; an activity she engages in "to feel alive," and can readily be perceived as an outward attempt to show the inward pain she should be feeling. The decision to kick both habits is a step toward maturity, the decision to stop unnecessary pain and a willingness to accept normal feelings and responses.
